A man is sitting on a bench outside at the end of a walkway, he looks sad. His dog is lying beside him with a tennis ball in front of him. A woman calls his name, "Mr. Randell." The man returns to the house. In the dining room, there is a business meeting going on. The man doesn't really listen to the people at the table and appears arrogant when he tells them his decisions. He ends the meeting partway through, which upsets the rest of the staff. They interrupted their day, drove through town to get to his house only to complete half their business. The man doesn't care. He heads upstairs into his son's room and talks with the doctor. The doctor says the diagnosis is not what he thought. He recommends the father get the boy to the hospital. The boy, Jack, wants to know if he is going to die. His father, Roy, tells him no. When Jack asks how he knows, Roy says, I"m always right aren't I?" The boys smiles and agrees.
Roy meets with Cuddy in her office. He wants House on the his son's case. Cuddy tells him House isn't available. He offers to send his private jet to pick up House wherever he is vacationing. Cuddy tells him House isn't on vacation. "In Jail? In Rehab?" Roy asks. Cuddy tells him his son will be in good hands with Foreman, Chase and Cameron. Roy says his son has already been in good hands and he's still dying. He is through with good, he wants the best and everyone says that is House. Unless House is dead or dying, Roy wants House treating his son, today.
Cuddy goes into House's office to meet with the team. She has already told House about Jack. She says the patient will still be Foreman's but House will be in the lead. Cuddy tells Foreman to not let House do anything crazy. "When is he getting his license back so we can stop playing this game?" asks Foreman. "Maybe never. This game is fun" House replies. Cuddy tells Foreman he has to present the DiBala case. Chase argues that they have more important things to do than satisfying everyone's idle curiosity. Cuddy tells him it's not idle curiosity. She leaves them to hash out Jack's case.
Cameron begins with the boys symptoms: abdominal pains increasing in severity and frequency, fever, dehydration, diarrhea and weight loss. Reading the file, House says the boy has seen 17 doctors. They need to rerule things out. He tells them to get a new history and physical on Jack. When Foreman looks at him, House says, "Unless that is crazy, Boss."
Cameron goes to Jack's room and conducts a physical, with Roy present. She asks if they have been out of the country or camping recently. Roy says they went to Montreal in January. Jack tells her they went to an all-star game for his birthday. Roy asks where House is. Cameron replies, "He's in his office. he's more of a decider than a doer. He wants us to start over because you never know what previous doctors may have missed. Like this..." She pushes on the boy's belly and asks him if it hurts more. He says no. When Roy asks what it is, Cameron says she doesn't know.
Back in the office, Foreman and Chase are going over the DiBala presentation. Chase tells Foreman to keep it clean, simple. "We did a test that called into question the initial diagnosis so they changed their minds," Chase says. Foreman wants to know what they should say if asked about the test. Chase says that it's a routine test. Foreman responds with, "A routine lab test doesn't involve blood you stole from a corpse you used to trick me into giving the guy the wrong treatment." Chase says, "Yes, and if you brought that up, it'd certainly make for a lively debate about the morality of murdering murderers." Chase tells Foreman that he burned the evidence and backed Chase up and nobody can prove anything. The body is buried 8,000 miles away. Foreman says, "The room is going to be filled p. Not because of a coin flip. Not because of a patient. But because House's department screwed up. They're going to be like sharks in a chum-filled swimming pool. And there's no way I'm jumping in unless I'm in a steel cage." Foreman wants to know that Chase has everything cover. From behind them, House asks, "What covered?". Chase stands up and tells House they were just going over the DiBala case and making sure they didn't miss anything. House says, "We didn't unless of course the thing that kill him."
Cameron comes in and shows the team Jack's x-rays. She says that he is constipated. Chase says, "Hirschsprung's disease - a congenital disorder of the nerves in the colon, very rare in a boy his age." They have to give the boy a barium enema and do a biopsy in order to confirm their diagnosis.
House sits down at a table at an outside cafe and says to the woman behind him, "Misty May digs for gold, put it in quotes." Thirteen turns around and tells him she's making travel plans. He wants to know if she's done with her little experiment with Foreman. She tells him it wasn't an experiment. She says she is glad House is better. She really liked working for him but she's not coming back. They exchange a little chit chat about her coming back. House gets up, begins to walk away and tells her to enjoy Cabo. She tells him she's going to Thailand.
Cameron reviews the x-rays they've done on Jack over the course of over a week and they show his colon is clear. Roy is thrilled his son is pain free for the first time in months and Jack says he's starving. Cameron checks Jack's belly and says while it feels better, it appears larger. Just then, jack begins to have a seizure. The treatment they give him doesn't work so they rush him into surgery to release the pressure in his brain before he suffers irreversible brain damage. As they hook Jack up in the O.R., Roy finally gives his consent. Chase, who is prepped and ready, drills a hole in the boy's brain, which relieves the pressure. Cameron goes out to tell the father, while Chase asks what happened. Foreman has no idea.
After the procedure, the team realizes Jack has not had any more seizures. However, he is continuing to build up fluid in his abdominal and dural cavities. Foreman asks the team if they notice anything odd about the shape of the fluid collection. House says, "Oh god, the Virgin Mary." Foreman points out the shape is "off" so House recommends they do a dural biopsy to rule out brain cancer.
In the backseat of a taxi, Thirteen is calling to confirm her plane reservations. The driver asks how long she's going for. She hesitates and says she doesn't know but is very lucky that a friend of hers is staying at her place while she's gone. She has a big dog that needs to be walked and fed. The drive tells her that he has a job, he's not going to rob her. Thirteen replies, "We're strangers. I don't know if your honest or dishonest." He tells her it's a nice way to live her life and calls her a *****. She says she apologized. Thirteen gets someone at the airline and tries to use her frequent flier miles to upgrade to business class. The driver tells her that obviously she is a stranger so welcome to his car. Thirteen is upset to learn she is not even booked on the flight and informs the airline it's their problem, not hers. She tells them she is on her way to the airport, she had already confirmed her reservations the night before.
Outside Jack's room, Chase and Cameron tell Roy about the possibility of his son having brain cancer. Cancer tells him there's no visible signs of a mass so it's possibly they caught it early. The father sarcastically replies, "Oh so your telling me him having cancer is a good thing?" He apologizes to Cameron as he signs the consent form. Chase gets paged to go and help Foreman prep for the biopsy. Cameron says once she's done getting Jack ready, she'll join them. Chase tells her not to. She feels she should stay at the hospital if he is. He tells her to go home and get some sleep because he's going to wake her when he gets in so she'll need the rest.
Thirteen shows up at Wilson's. House opens the door and she tells him to stay out of her life. She accuses him of spying on her and then canceling her plane reservations. House says he was talking to her, but spying on the MILF in shorts. She believes he sabotaged her reservations to get her back on the team. House says that with her out of the picture, Foreman is working better than ever, so why would he want her back? House says it doesn't matter which island in the Bahamas she is going to be in, or Cabo. She says she told him she was going to Thailand.
In the office, Foreman informs Chase that when Cameron ran DiBala's blood, the printout he got was only what tests Cameron ordered. The lab did a full work up and Chase didn't check the man's cholesterol. Foreman informs a speechless Chase that he was off by 20%.
Cameron is sleeping in bed and her alarm goes off. She wakes, turns it off, then rolls over to find Chase never came home. Meanwhile, Chase and Foreman are at the hospital going over the DiBala case. Chase feels tat DiBala didn't die of high cholesterol, so why would anyone check. He mentions maybe Cameron calculated the numbers incorrectly. Foremen doesn't want to stack lie upon like. Cameron walks up and asks Chase where he was. He apologizes and says he got tied up. She's a little upset he stood her up when he said he'd wake her. She tells them Jack's biopsy results are in and it's not cancer.
Sitting in the office, Foreman asks, "How can so much be wrong with nothing being wrong?" Cameron suggests Adno Carcinoma of the stomach, which would cause all of Jack's symptoms but not show up on the CT. Chase stays quiet. Foreman sends Chase and Cameron to go scope Jack and get another set of biopsies. When they leave, House accuses Foreman of canceling Thirteen's reservations. Foreman doesn't know what House is talking about and feels he has done all he can with Thirteen, all he has left is to move on.
Leaving the office, Cameron quietly asks Chase what is wrong. Chase is defensive and denies anything is wrong. She tells him he can't hide it from her. He finally tells her there is a discrepancy in the DiBala blood panels and asks her to not tell Foreman he told her.
Jack remains unconscious while Roy stands there and blames himself. He tells Cameron and Chase that he inherited his business when he was 24. How he has made major deals which have resulted in a lot of money. He wonders why everything he does, everything he touches turns to gold, except his family. First he lost his wife, now his son. He feels it's his fault, it's karma. Cameron tells him it's not true just as the boy begins to seize again. When they check Jack's readouts, they realize it's not inter-cranial pressure.
House stands in the hallway, looking at Jack and his father. Chase says the seizures have stopped but Jack hasn't regained consciousness. There is no sign of masses in Jack's stomach or esophageal lining and he's still accumulating subdural and pertinal fluids. The tests are ruling out cancer. Chase thinks it's an infection but all the tests are coming back negative. Cameron wonders if it's abdominal epilepsy and the seizures would look like pain and protein malnutrition would cause fevers. Foreman looks at House and asks if he's certain. House says, "I'd be a lot more certain it if was my idea. It's the best we got." Foreman sends Cameron in to begin antibiotics. Chase runs after Foreman to talk about the DiBala case. He recommends Foreman state that Chase prescribed something but didn't chart it. Foreman figures Chase is going to accuse the pharmacy of bad bookkeeping until Chase suggests they say he just went up and took it out. Foreman walks away and says he'll handle it.
Foreman goes into Cuddy's office and tells her he can't present the DiBala case. He tells her he doesn't have time, House is pawning his responsibilities onto Foreman. Cuddy reminds Foreman that he wanted House's job and the responsibilities are his, not House's. Foreman says, "I wanted to be in charge, not a figurehead." He argues that he doesn't want to do the presentation but Cuddy holds firm. Foreman comes out of Cuddy's office and Chase is waiting. He wants to know if it worked and Foreman replies, "Not even close."
From a seat in the hallway, House is watching Jack and Roy. Cuddy walks up to see how he's doing. She informs him that Thirteen called and said someone hacked into her email and canceled her reservations. Cuddy thinks Foreman did it. House is surprised that she's not accusing him. Cuddy wants to know if he still believes Foreman should be in charge. He tells her that Foreman loves power, he loves puzzles. Cuddy says, "You love power and puzzles." House says, "But when I had both..... It wasn't Foreman because he said he'd moved on." House's beeper goes off so he says it's time to get off the bench. He walks into Jack's room and Roy realizes he's House. House says, "I hear you're a fan. I'll have my guy send over a signed glossy." Cameron paged House to tell him Jack doesn't have abdominal epilepsy. She shows him a rash on Jack's chest. House says, "I didn't expect that. Damn it, I knew I should have stayed outside. Fantasy is always so much better than reality." The father asks if House thinks it's a joke, his boy is dying and neither of them know why. "I know. That's why it's so interesting," House says.
Pacing in the office, while the team sits at the table, House reviews Jack's symptoms. Foreman says it could be an allergic reaction to the meds, which House rules out. Chase comes up with
"Polyarteristis nodosa." Foreman challenges everything Chase says. House agrees with Chase and Foreman looks at him. House says, it's not his decision. They should start Jack on prednisone and get a testicular biopsy - if Jack has any real inflammation, it will be down there. Foreman says he'll do it with Cameron. Cameron asks what is going on. First he wanted Chase to help him with the prep. Foreman claims nothing. House asks Cameron if the rash is on Jack's penis. He says it's most likely affect the small blood vessesl which explains why they didn't see any inflammation on the biopsies. He says it's not polyarteristis nodosa, it's Degos Disease. When the team looks at him, he's says he'll do it.
House goes in to see Roy. He tells him Jack has Degos Disease. Roy wants to know what the treatment is and House tells him it's incurable. Roy insists House is wrong, he's already been wrong a bunch of times. House rechecked all the biopsies. Roy wants to know how long Jack has. House says, not much longer, a day at the most.
House is grabbing his stuff from the office and sees Chase in turmoil at the conference table. Chase gets up and goes to Cuddy's office. He tells her that Foreman can't present the DiBala case, it's not the time. Cuddy tells him she knows there busy but it has to be done. Chase starts to tell Cuddy the truth when they get interrupted. Cuddy is called to pediatric ICU by a bunch of lawyers. Before she goes she tells Chase that if they screwed up to just own up to it, it can't be that bad.
In Jack's room, Roy is standing there with his lawyers. They call him insane. House says he's irrational, not insane. Roy believes it's karma and if he gets rid of everything he has, it will help his son. The lawyers inform him that he will be bankrupt and thousands of lives will be affected. Roy says Jack isn't dying, he's not going to let him. He signs the paperwork to divest himself of his business and put people out of work. He made himself a poor man because he believed he could save Jack. Just then, Jack flatlines.
Thirteen shows up at Wilson's office. She thanks him and wonders if it was worth breaking the law. He says she's welcome but has no idea what she's talking about. Cuddy had the tech's chase IP addresses to find out who access her email; only her's and Wilson's IP addresses showed up. He tries to say it was House then admits he did it. He says House needs someone that doesn't need him. She's the only one who doesn't feed into his crazy vortex, she keeps his ego in check. Wilson tells her it's a great job, maybe even better now that House is at least trying to have real relationships with people. He tells Thirteen to not throw it all away due to a bad breakup. Thirteen says goodbye and walks out the door.
Wilson goes to House and says he confesses. House says he knows he didn't do it so why did he confess. "She's a smoking hot, newly single bisexual, " House says when Wilson asks why he doesn't want her to go. House feels Thirteen is making a mistake as she's a good doctor. House starts to tell Wilson he was born with a heart three sizes too small.... he heads in and finds Jack is still alive. Foreman comes out and House tells him it's not Degos Diseases. Jack has obstructed blood vessels and they can't be obstructed because of Degos. He says it's Antiphospholipid Syndrome. Foreman asks if he's sure. House says he isn't but will be if the boy responds to treatment. House says, "By the way, someone screwed over your girlfriend. She ran to me, Cuddy and Wilson. Everyone but you. She's not over you yet or she is..."
Chase reopens DiBala's file and finds some test results sitting on top. He goes to Foreman and brings the document. It shows that one of DiBala's doctors was prescribing medicine to increase the HDL #'s. Neither Foreman nor Chase requested the information, which means someone else knows.
When House awakens the next morning, the business section is on the arm of the couch. He opens it to read that Roy has declared bankruptcy. He goes to the hospital and stops by Jack's room. He finds Jack sitting up in bed, sharing a pizza with his father. Both father and son are smiling and happy. Roy tells House, "It worked." House says the medicine worked and then leaves. Jack and Roy just smile at each others.House walks into the office and Chase is sitting at his desk, playing with his tennis ball. Chase wants to know how House knew. House explains that DiBala was a large man, it's only given that he would have cholesterol problems.. They talk for a few minutes and Chase tells House, as he hands him his tennis ball, that he will always be in charge.
Cameron walks up, turns off her alarm and rolls over. Chase didn't come home again. He's sitting in the auditorium listening to Foreman present the DiBala case. Meanwhile, Thirteen arrives at the airport and boards the plane to Thailand. Roy's house is up for sale with a "bank owned" sign in front of it. House sits in the lounge chair in his office......
